Job Title: Physical Therapist - Per Diem
Job Description: The Physical Therapist is responsible for the evaluation, planning, directing, and administering of physical therapy modalities of treatment as prescribed by a licensed physician. Administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, surgery, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities in the home setting.
Job Prerequisites: Current state Physical Therapy licensure, graduate of an approved School of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.

Recommend a minimum of one (1) or more years of previous experience in all aspects of physical therapy.

Current BLS (CPR) certification.
Job Responsibilities: Performs an appropriate assessment per CMS guidelines for each patient assigned. Completes reassessments as per policy. Completes discharge OASIS as needed per agency policy. Communicates with members of the home health treatment team on patient progress issues.

Observes and evaluates effect of treatment. Recommends changes to physician, if needed.

Formulates a treatment plan based on identified needs. Includes family/caregivers in teaching as appropriate.

Demonstrates knowledge and proficiency of physical therapy modalities.

Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family. Has awareness of available community resources.

Manages and operates equipment safely and correctly.

Communicates appropriately and clearly to physicians, facility staff and other members of the team.

Coordinates and directs patient care to ensure patients\\\' needs are met and home health policy is followed.

Maintains records pertinent to the personnel and operation of the department.

Demonstrates an ability to be flexible, organized and function under stressful situations.

Maintains a good working relationship both within the agency and with other hospital departments.

Participates in performance improvement activities for the department and organizational QI/UR activities as directed.

Meets current documentation standards and policies.

Coordinates, delegates and supervises responsibilities assigned to supportive staff CNA, PTA, etc.).
